Admission Approval Process

Adventist Aged Care has a 3 steps Admission Approval Process

To start our easy admission approval process we need the following information from you. Completion of this information will help us to start the admission process and wait list as soon as possible.

1. Copy of your ACAT – Assessment of Eligibility – This assessment may be organized by contacting your local GP or by calling the following toll-free number 1800 200 422.

2. Copy of your “Assets Assessment” from Centrelink – Line FreeCall 1800 227 475

3.  Download and complete our free information pack

A copy of the Centrelink “Combined Assets and Income Assessment Form (SA457)”  this is not compulsory but essential for us for fully or partially supported residents and provides information to ensure the correct fees are charged upon admission.
